Five Tips for Dealing with Nausea from Medication

Are Meds Making You Sick? Here's How to Manage Nausea

By Donna Rae Siegfried

Treatments for arthritis have come a long way. Often they reduce pain and swelling, slow the progression of disease and minimize permanent damage to joints. But nausea from medication is a problem. The top offenders? Disease-modifying antirheumatic drugs (DMARDs) azathioprine, cyclophosphamide, leflunomide and methotrexate; all nonsteroidal anti-inflammatory drugs (NSAIDs); and antidepressants such as duloxetine or venlafaxine, say rheumatologists. To combat nausea from medication you take for arthritis, follow these steps:

1. Talk. Discuss the severity and frequency of your nausea with your doctor. He may be able to alter the timing and dosages of your medications to reduce the unpleasant feeling or prescribe a nausea medication to help.

2. Take. If your doctor has prescribed nausea medication for severe cases, take it as soon as you begin to feel nauseated.

3. Eat. Instead of three large meals per day, spread your day’s food over five smaller meals. Choose easy-to-digest foods and stop eating before you feel full; overeating can increase nausea. Before taking your medication, try nibbling on a few saltines.

4. Drink. Avoid drinking too much liquid with your meals, and drink slowly between meals to prevent triggering nausea. Sip ginger ale or lemon water to reduce nausea.

5. Rest. Let your stomach settle after meals. Avoid vigorous activity for 30 minutes or so, but do not lie down while you’re waiting. Read a book or magazine to keep your mind occupied.

I have been on methotrexate by injection once a week for the last 10 years. I always felt a bit nauseated for the following 2 days after my injection but it never bothered me too much. I noticed for the last two months that I'm nauseated all the time. I don't know what's going on. I'm worried and I was wondering if this happened to anyone else.

My arthritis hit me about 2 1/2 months ago. All of a sudden my knees started to swell up, chills, tired, felt like whole body had been run over by a train. The fevers are gone but none of the rest. I had had sore hips for about 2years prior. Now all my joints are sore and swollen. I am seeing an RA specialist. They have me on 3 drugs, Sulfasalazine, Naproxen, and Methotrexate. Started the Sulfasalazine about 1 month ago. Started the Naproxen 11 days ago. Started the Methotrexate 7 days ago. Didn't really have nausea with the Sulfasalazine or the noproxen, but took the methotrexate on Sunday and then on tuesday morning at about 1 am in the morning started feeling nauseated and it stayed with me all day tuesday. Felt wiped out tuesday as well. Wednesday felt fine (sort of) then thursday felt nauseated again. Friday felt fine (sort of) in morning then afternoon and evening not so great. Now today is saturday and I get to look forward to taking my methotrexate tomorrow. I wonder what my week will be like? It is early days yet, I am new to this disease and how to handle it. I am grateful for all the info here. I will be trying the ginger, I think that is a good idea. Good luck to everyone here. I hope they find a cure for this disease soon as I have to tell you, I am not having any fun with this. I guess Forest Gump's mom was right. I've been taking 10 pills per week for over 13 years. About 6 years ago, I started getting the injectable MXT, and just drink the liquid mixed in with a little water. It doesn't have any taste, and it is a lot cheaper. About $18.00 for a 10 week supply.

Also, I have found that for me it is best to take it as soon as I get up in the morning. I drink half a glass of water with it, and after I'm ready for work or play, I'll eat a lite breakfast. I feel a little puny throughout the day, but it's a minor inconveniece. Sometimes I don't feel bad at all. I think that by taking it then, and having activity afterwards, it helps to get it through my system and the rest of the week is all good!

Take injectable Methotrexate, 12 years now, use smallest, thinest insulin needles I can find. Also on Humira. Not too much stomach problems, however need to watch what I eat. Occassionally take Pepsid complete. Acidy, spicy foods, coke, soda, coffee can't tolerate. Use to have gastro problems, when I was on pills. "Any" kind of pills, & they give you so many when you first get diagnosed. Cortisteroids & methotrexate together, show caution, not good to mix. Now have ankle sprain so have taken small dose of ibroprophen. When stomach bothers me switch to tylonel. I avoid pills as much as possible. Vitimins can cause gastro distress too, however, some, like vit. E, flaxseed or krill oil don't. Vit D does not, but it revs up my immune system. Selenium goes down well. Chewable calcium o.k. Lethicin easy on stomach.
You learn over time what works for you. Organic food best. If I get a sinus infection etc... stop the meds, start the mild salsa (tomatoes rev up my immune system) - (peas have lots of folic acid,) take vit D & infection gets better.

Problem is so much tainted food & food recalls now out there. Am "learning" to deal with that. Tap water has cryptospordium in it. I boil all drinking water before putting in frig, or zap it in microwave. Oh heck, could write a book here. Sometimes being on the front lines in the sense that you have the disease, you know how your body reacts or you learn, you are the best one to figure out what works & does not for you. You need to be the creative one to come up with ideas, you have to be the one to figure out what to do, what to put in your mouth & what not, to yourself. Others can be of help, but in the end you have to listen to your own body, study up on things & etc...

Hi, I am so glad to see this real discussion on Methotrexate. I have been on 10 pills weekly for 5 years and am now down to 6 pills and take Remicade IV every 5 weeks. I am doing great. I take my 6 pills on Sunday after lunch. I still have some nausea, headaches diareha and dizziness. My rheumie says that dizziness isn't due to the Methotrexate, but I know it is. It has gotten better over the years-stick it out as it is worth it! Jan

I started taking 6 methotrexate pills about 8 weeks ago and it has really helped my swelling and stiffness. At first I tried taking it at night after dinner, but was knocked out the whole next day because it interfered with my sleep. So I'm trying it in the morning this week. Does anyone else have problems with methotrexate and sleep quality? Thanks.

I've been taking 6 x 2.5 mg of Methotrexate once weekly and have been wearing a seasick band for the following 3 days. So far I haven't suffered from nausea, but I do get very tired for a couple of days after I've taken my Methotrexate. Then I have a folic acid supplement 5 days later and this seems to help with any other symptoms.

I had a very painful mouth with blood blisters but don't get these any more since using a toothpaste for sensitive teeth. Hope this will be helpful.

I also was on methotrexate pills and due to stomache issues doc switched me to injections. I take 25 mg injection one time a week. I just yesterday noticed a little dizziness but almost never have problems. I was in a flare for last 2 days then took weekly methotrexate and now flare is much better. I am also on plaquenil 200 mg 2x day. Soon I will be starting Arava. It is wonderful to find a medication that works and scary when it no longer is working for me.

I had to stop taking the methotrexate pills because of the vomiting and intestinal problems. Was ill for 3 days after I took the pills! My doctor switched me to the injections, but the room spins and I am in a fog for a couple of days after. I am still having a difficult time tolerating Methotrexate. Hopefully, when I switch to Remicade that my doctor will let me stop the
Methotrexate, as it doesn't seem to work very well. I have been taking Methotrexate for 6+ years now.

The methotrexate nausea was horrible, even
after switching to the injections I lost 2 dats
a week to nausea. I usually had an 18 hour
to 24 hour window before the nausea set in.
I recently started on Rituxan, after the first
infusion I had teeth chattering chills for2 hours
I felt better for a few days and then the pain
returned. I had the 2nd rituxan infusion a week
ago today. I am starting to get some relief although
I had to have two corta steroid shots for my
right knee and hip tests because I could'nt
stand or sit comfortably or even sleep. The not sleeping
was bad since my 2nd infusion I have been so
exhausted it's unreal I am sleeping 12 hours
a night and napping during the day.
I am told this should pass I'm the next few weeks.
I am hoping along with the arava and plaquenil
to finally get into a remission. even being in
steroids much of the time since January, I have lost
60lbs. Food either has no taste or just tastes bad.
I wish someone would figure out how the
high level antibiotics could be used long term
and safely for RA. The best I have felt RA wise
was for 6 weeks( 4 weeks on) and two weeks
following after being on Vacomyacin, however,
that was the result of 2 weeks in the hospital ( and 2 weeks of round the clock IV therapy) for MRSA
pnuemonia and bacterimia.

I am just hoping to stay healthy until I can
get my flu and H1N1 vaccines

MRSA pnumonia, and a blood infection

I take 8 methotrexate a week for my RA along with dicfleconac for inflammation due to my RA and fibromyalgia. I am taking other meds for the fibro also. I notice that I have nausea and dizziness and the following day I feel compeletly worn out so I try taking it on Saturday mornings and then am tired on Sundays and can nap after church. I used to be so entergetic but now get tired easily and I teach elementary so I stay worn out. Does anyone know anything to help with energy?
I still ache when the weather is cold or rainy. I will keep praying for everyone here. It's good to know others understand how it feels.

I take methotrexate 4 pills once aweek and I get very nausea for days. My doctor gave me Phenergan to take 1 hour before I take my methotrexate. So I now sleep all day the frist day.And feel better the rest of the week. I know taking meds for the side effects for another one is not the best thing,But it works great for me.

I take 6 methotrexate 2.5 milligram tabs once a week along with every two weeks a 40 mg. shot of humaria and whenever needed I take celebrex only one though when needed maybe twice a week tabs. I have noticed with the methotrexate I can get episodes of dizziness meaning the room spins and it is just an unnerving feeling. It alwasy happens at night when I lay down though! This does not though happen every time I take it just occasionally its been happening only like twice a month now! I think my body is adjusting better to this now! I was on 10 pills and have been gradually dropping down since the humaria is doing a good job for me!

I have to travel international for my work and I am worried about timing to take my medication. Do I take it same time as used to take it at home or switch to new times?
I am traveling to a country that is 6 hours ahead of us.
I take methotrexate (once a week) and high blood pressure medications (daily).

Any Clues ..

I did not have much nausea with methotrexate but I get alot of dizziness. I noticed the difference when I got off. It was like I just got off a carnival ride and had to hold onto something while I walked for a few seconds. Going into a study where I don't need to be on methotrexate for my RA. Study by Pfizer. Let's see how I'll feel then.

I have been on 25 mg Methotrexate for over 20 years for my RA and nausea was always a major issue. I switched from pill form to injections about 2 years ago and my nausea has improved from 3 days a week every week to maybe 3 or 4 times a month. I'm also on Orencia and predisone for the RA. My Dr. gave me a scrip of compazine to take for my nausea, it really worked for me.

I take methotrexate 10mg twice a day once a week, and Sufasalazine 1000mg twice a day, every day for RA.
I take them after breakfast and supper with no nausea.

I tried Hydroxychloroquin 200mg twice a day, but the ophthalmologist noticed my vision was affected, so I stopped.
I also tried Minocycline 100mg, which made me nauseous, not severe, but most days and most of the time. I finally stopped that when I went on vacation in August.

Hope this helps.

I take the methotrexate by shot once a week. I am nauseous the next two days. I also get Orencia infusions every four weeks and that give me nausea and headaches. I take Zofran for it, but my insurance company is stingy, only 12 tabs a month. I do have morphine for the pain, but that also upsets my stomach. I take Previcid 24 to help with all of this and it help quite a bit, but not enough.

I drink a lot of ginger ale and eat candied ginger. Saltines are all I can eat some days. BUT I am getting relief from my RA, Fibro and spinal spondylosis (sp?). So it is a trade off. my finger are starting to return to thir normal shape and I don't hurt nearly as bad for nearly as long in the morning.

I take Methotrexate once a week for SLE(systematic lupus enthrombosis).Nausea is a problem then.I also take the generic form of Plaquinal. It's not as bad,but I don't have the appetite I used to anymore. Also, every time I do eat, within 20-30 minutes I'm in the bathroom with diarreaha.I had my gallbladder removed 7years ago, could this have something to do with it?

I take multiple medications for multiple condtions so nausea is a constant issue. My doctor prescribes both an anti-nausea rx ( Zofran) and Klonopin( an anti-anxiety rx). My doctor explained that the anti-anxiety rx is given in the ER for patients with extreme nausea and sometimes when given when you first start feeling nauseated it will help to prevent vomiting. I have found it to be very helpful. It also helps me to be able to sleep when this occurs in the evening.

I have not had stomache issues with remicade, but I get reallly exhausted after my infusion. My nausea is weird, it can be so sudden and I think it is the plaquenil and percoset. I do always have crackers on me, but sometimes like today, nothing works.

Even before I was on Methotrexate, I would relieve my nervous stomach by putting about 3/4 tsp. of gr ginger in my hot green tea. Let it seep about 3 min. w/cover on and sip slowly. This has become such a habit that tea does not taste good unless I have ginger in it. Also calms a bad cough due to a cold. For that you can increase the ginger to 1 tsp. & can add honey.

I have almost gotten rid of nausea by taking 1/4 tsp. powered ginger mixed with the water I take to swollow my meds. I buy the ginger in bulk from the local health food store so it is very inexpensive. If the taste bothers you ginger is widely available in capsule form.

When I was on methotrexate I found that placing a small amount of ground ginger on my tongue helped relieve the nausea.
