Corticosteroid Benefits Without Side Effects

Researchers work toward pill to replace corticosteroids like Prednisone.

Imagine a medication with all the anti-inflammatory power of a corticosteroid but without corticosteroid’s detrimental effects on the bone or other side effects. Thanks in part to work by Arthritis Foundation-supported researchers, such a drug – using a protein called glucocorticoid-induced leucine zipper (GILZ) – may one day be available.

In a study reported in the April 15, 2008 issue of the Journal of Cellular Biochemistry, Xingming Shi, PhD, and his colleagues at the Medical College of Georgia in Augusta found that the GILZ protein mimics the action of corticosteroids and blocks signals from inflammatory cytokines such as tumor necrosis factor-alpha (TNF-a) and interleukin-1 (IL-1), known to play a role in the inflammation and destruction of rheumatoid arthritis and other inflammatory diseases.

The advantage of the GILZ protein over corticosteroid medications is that it could potentially prevent the bone loss that is often a side-effect of long-term steroid treatment and also might not create the stores of excess fat common in people who take those medications, says Shi, whose research focuses on the cellular and molecular mechanisms of corticosteroid-induced osteoporosis and the search for new treatments of the problem.

The reason is that both bone and fat are derived from mesynchymal stem cells.

Traditional corticosteroids shift the balance between bone and fat production and cause more fat cells to accumulate. Excess fat leads to more cytokines ­– chemicals that signal for inflammation, which stimulate production of bone-destroying cells called osteoclasts and inhibit bone-forming cells called ostetoblasts. Permanently expressing GILZ, however, prevents mesynchymal cells from differentiating into fat cells and instead increases bone formation.

Shi already has developed a cell line that continuously expresses GILZ. Eventually, he would like to develop a GILZ pill that could be taken in place of a traditional corticosteroid.

I am on 10 mg daily, as side affects has stopped other treatment for R A, and the new drugs on offer can damage the lungs, I am concerned about the long term use of prednisone and would welcome GILZ instead.

I would love to take part in a clinical study of GILZ in the Boston area. Please contact me. Any predictions are when this new medicine will be ready? I've been on a maintenance dose of 5 mg prednisone/day and fear for the long term consequences.

Is there a clinical trial currently being conducted for this medication? Any idea as to when a presciption for this medication may be available?

I was diagonised with PMR 6 years ago. I have been on various does of steroids since that time. I am now on 5 mg. I have all the side effects, hair loss, weight gain, cataracts,etc. I so long for the day a new drug is available to me.

my mother is 75 years had her hip replaced and has been a RA patient for 25 years. She needs some kind of medicine which can relieve her of the pain and agony she's been going through. Do you think GILZ is the answer? are there any of your agents in Karachi Pakistan

I have been on steroids for over 25 years for RA. Are there studies available? How can we get more information? I am desperate for an alternative solution as I have not been able to get off these horrible drugs. PLEASE send any news. Thx.

I have been on 20mg of prednisone daily since October 2008 since I was diagnosed with PMR under control. I have gained weight, 15 pounds, and girth, size 10-12 to 12-14. How soon will this medication be available? I have bruises all over my body along with very thin skin. I am 75 years old and really would like to be "normal" again. During this time I have also been diagnosed with sleep apnea. and am using an auto-bi-pap machine. Do you find these symptoms can be inter-related?

I have been on steroids for 20 yrs. (10mg.)
per day. I would welcome GILZ as a replacement. If you are doing studies in the NY area please comtact me.
