Arthritis and Diseases that Affect the Hand and Wrist

Many forms of arthritis and related conditions that affect the joints, muscles and/or bones can cause problems like pain, stiffness and swelling in the wrist and fingers. Other conditions can cause additional problems, such as numbness and tingling, pitted nails, painful ulcers or thickened skin that makes bending the fingers difficult. Here are some possible disease-related problems that affect the hands and wrists.

  • Osteoarthritis (OA). The most common form of arthritis, osteoarthritis is a chronic condition characterized by the breakdown of the cartilage that cushions the ends of the bones where they meet to form joints. This breakdown causes the bones to rub against each other, causing stiffness, pain and loss of movement in the joint. In the hand, the joints most commonly affected by osteoarthritis are the wrist, the joint at the base of the thumb (the basal joint), the joint in the middle of the finger (proximal interphalangeal joint or PIP) and the joint closest to the nail (distal interphalangeal joint or DIP). In the finger joints, OA can lead to the formation of bony knots. In the PIP joint these are call Bouchard's nodes. In the DIP joint they are called Heberden's nodes.

  • Rheumatoid arthritis (RA). Rheumatoid arthritis is a chronic inflammatory disease of the joints that occurs when the body’s immune system – which normally protects us from infection – mistakenly attacks the synovium, the thin membrane that lines the joints. The result can be joint damage, pain, swelling, inflammation, loss of function and disability. Rheumatoid arthritis commonly affects the wrist and finger joints and can cause deformities that make it difficult to use the hands.

  • Juvenile arthritis (JA). Juvenile arthritis is the term used to describe arthritis when it begins before age 16. There are several different types of juvenile arthritis that can cause pain and swelling in the wrist and joints of the hands.

  • Gout. Gout is a form of  arthritis that occurs when excess uric acid, a bodily waste product circulating in the bloodstream, is deposited as needle-shaped monosodium urate crystals in tissues of the body, including the joints. For many people, the first symptom of gout is excruciating pain and swelling in the big toe – often following a trauma, such as an illness or injury. Subsequent attacks may occur off and on in other joints, including the wrist and joints of the fingers. After years with the disease, lumps of uric acid, called tophi, may form beneath the skin of the hands.

  • Reactive arthritis. Reactive arthritis is a chronic form of arthritis that often occurs following an infection of the genital, urinary or gastrointestinal system. Features of reactive arthritis include inflammation and swelling of the joints, eyes and structures within the gastrointestinal or genitourinary tracts, such as intestines, kidneys or bladder. A small percentage of people with the disease develop a rash or hard nodules on the soles of their feet or palms of their hands.

  • Lupus. Lupus is a chronic autoimmune disease, meaning the body's immune system creates antibodies that attack healthy tissues, including the joints. The wrist and small joints of the hands are among those most commonly affected by lupus. Lupus can also cause inflammation in many organs, including the skin, heart, lungs and kidneys.

  • Psoriatic arthritis. Psoriatic arthritis is a form of arthritis accompanied by the skin disease psoriasis. The skin disease often precedes the arthritis; in a small percentage the joint disease develops before the skin disease. The joint involvement of psoriatic arthritis often causes inflammation of the entire finger, giving it a sausage-like appearance. Approximately 80 percent of people with psoriatic arthritis experience changes to the nails including pitting, thickening and/or separation from the nail bed. The skin rash of psoriatic arthritis can also affect the hands.

I have a new lump on my thumb bone, (joint closest to nail). Its like a hard BB. Also joints in thumb and fingers seem to be getting bigger!. This is happening really fast! pain in wrists. (I play golf).
Pain/swelling in ankles/feet. Can arthritis happen this fast? a few wks./months? THanks

I have pain and stiffness in my right wrist,
hand, and all the fingers, my left thumb has
a knot sticking out of the bottom joint, that
hurts,and the top joint clicks when I move it. I also, have pain and swelling in
both ankles, right knee, right shoulder, lower back, & right hip. It has come on real
heavy in the last 3 months. If I sit very long I can hardly walk, everything is stiff, but loosens up as I keep moving. I have alot
of pain everywhere. What do you think I have?

Hello, I am 59 I have OA with degent.disk, bone spurs. Had total neck fusion front and back sides, lower back as well.I think 9 or 10 surgerys, both hands for carp. tunnel. Now my hands have started to swell on the knuckels of both hands every joint in my hand hurt to the point they wake me up at night.My feet and toes. I already take a group of meds, and have just went to county doctors. They are testing for Ra now.. If this is RA How do I slow it down, is it diet? It all seems to happen so fast.

i have pain in both hands up to, and including, my wrists. all of my fingers and both thumbs are affected. my hands itch. the pain wakes me during the night. the inside of both hands are developing cysts or knots. i have redness and inflammation. what can i do before i see a doctor for the condition? i am a 61 year old female.

Been told I have carpel tunnel in both hands. Left is mild with right being severe suggested I get surgery. Had the testing done seeing the orthopedic DR. the 24th. Any suggested questions to ask as far as other ideas other than surgery? Any types of shots to try first? I do have alot of pain in mynright hand and fingers. No wrist pain. My fingers get numb and they swell. Just using Tylenol Arthritis pain relief. and some pain rub. Very annoying right now. I am a nurse assistant and use my hands all the time. Thank you for your time in this matter.

Fingers are twisting in different directions, numbness and pain traveling up the right arm. Hand is starting to curl to make cup. What can I do?

RH in left hand fingers and joints. Took them 6 months to diagnosis. I've always been extremely active as far as running and lifting weights. . . still running and lifting lighter weights. With RH, I believe it is a mental thing for me to stay active and exercise. Does RH mirror itself on the opposite side of the body? just curious.

had carpal tunnel both hands yrs ago, had surgery, then got the cycts on wrists, endometrtious b4 hysterectomy, now dealing with RA, GOUT, SLE, so tired of being sick and tried and in pain....

I was diagnosed with carpel tunnel by two doctor but after handing the nerve conduction study done was told that I only have mild left neuropathy, locate at the elbow. I am waiting for my followup appointment with my doctor to discuss the options. I also have cysts on my right palm on the tendons. My biggest complaint is the pain in both of my wrists. I am not sure what the diagnosis will be but hopefully something will help with the pain.

ive always suffereand pain in my back and tops of my legs and last couple of years its started in my hands and wrists mainly of a morning when i cannot move my back at all sometimes but past few months ive had this pain in my left hand and swelling to me its as tho its something to do with my vein like a clot or lump and my veins become extremely visible through my skin also sometimes when i hold anythung made of metal ie mobile or wear a bracelet silver watch it seems to throb more ive never been diagnosed as even having arthiritis as ive never complained with it until recent but this blood/vein is now starting to worry me any ideas what this could be?

I have osteoarthritis in my fingers with Bouchard's nodes. My doctor took me off diclofenac because of heart issues and put me on naprosyn. It does not work as well, occasional upset stomach, some hair loss. The nodes are very painful and some of my middle joints are frozen, which is very disheartening. Wondering about any other comments or solutions? Thanks!

All of the knuckles on my hands are getting tender and stiff, but the thumb joint at the base is the worse. I also feel like i may be getting a spur on the inside of that knuckle on my right hand.The main question I have s my hands itch so bad at the affected joints, is this also a symtom......thank you

I have had pain and swelling in all my joints at one time or another. Right now my left thumb wont bend. It is swollen, and the joint at the base is quite enlarged. This has been going on for months. My spine alone causes me chronic pain. What to do? Gotta live with it, I guess. I would love to know what kind of arthritis I have though.
