Injectable Methotrexate Recalled

Some vials of the drug, a mainstay of rheumatoid arthritis treatment, are tainted, posing a risk to users.

10/28/10 A drug maker is voluntarily recalling vials of injectable methotrexate after small flakes of glass were found in some of the medication. The company says this has the potential to damage patient’s blood vessels or lead to swelling and even death, although the company isn’t aware of any reported problems so far.

Methotrexate is a disease-modifying antirheumatic drug, or DMARD. It is one of the most commonly prescribed medications for rheumatoid arthritis, or RA, a disease in which an overactive immune system attacks the joints and other tissues and organs. Methotrexate is taken to slow the progression of RA. It is also used for juvenile arthritis, severe psoriasis and some forms of cancer.

Sandoz, the generic pharmaceuticals division of Novartis in Princeton, N.J., is one of several suppliers of generic methotrexate. The company is voluntarily recalling all of its 50mg/2mL and 250mg/10mL vials of injectable methotrexate that are part of the Sandoz and Parenta brands and have expiration dates ranging from December 2010 to March 2012.

“Sandoz is initiating this voluntary recall of all 24 lots of the affected product following the finding of small glass flakes by Sandoz quality control in a limited number of vials in four lots,” the company explained in a press release.

The U.S. Food and Drug Administration has been notified of the recall. The company is also alerting retailers, wholesalers, pharmacies, doctors and patients. The company says people should stop using the methotrexate they have immediately if it’s part of the affected lots. Donald Miller is chair of the pharmacy practice department at the College of Pharmacy at North Dakota State University in Fargo and a member of the Arthritis Today medical advisory board. He notes that injectable methotrexate comes in other doses, and arthritis patients may use smaller vials that aren’t part of this recall.

He also says that if patients have difficulty getting injectable methotrexate because of the recall, methotrexate tablets are a short-term option.

“If people are using the injections and they simply can’t get it, they can still take it by mouth. Most people take it by mouth in the first place,” Miller says. “Those who take it by injection usually have had some nausea with the tablets or they aren’t getting enough of an effect and they get a stronger effect from injections. But in a pinch you could certainly go back to taking it with a tablet.”

He says patients should definitely consult with their doctor about how to proceed.

Anyone who has a reaction to the medication should contact a doctor right away. The FDA can be notified of adverse reactions through its MedWatch Safety Information and Adverse Event Reporting Program. Click here to read the FDA's press release about the recall.

I know this doesn't make much sense, but, sometimes it is easier for the pharmacies to obtain 10ml vials with preservatives, then 2ml vials...go figure. The Walgreens pharmacists told me this. I had the doc write a script for 10ml vial instead of 2ml vials, and even though it took them about 2 weeks....they were able to get the injectable methotrexate in 10ml vial. Yay for my 8 year old that is batteling Juvenile Dermatomyositis!!!

I have been on MTX for 20+ years. I have called numerous pharmacies, and no one has a clue when it will be available.

Sue, If you still have not found MTX try CVS they now have the 2ml bottles preservative free.

I had to use 20ml and throw the rest away for a few weeks because I could not find it anywhere else...

With preservative free ALWAYS throw it away after using what you are prescribed. Re-using it can cause infections.

I still haven't seen a new recall.

I just found this site and became aware of this recall because I cannot find the injectable methotrexate I need anywhere. The last post was a year ago!! I have to take the shots since I have had bariatric surgery and do not absorb the oral meds. Anybody know where I can find a supply? What about the warning that the bottles without preservative can only be used once and discarded?? That means I am supposed to throw away enough for three more weeks when I am having so much trouble finding it!

I was on metho for a long time teling docs that is made me sick and vomiting and diareha all the time. No one believed me untill I just stopped. Changed docs now getting better treatment

How long have the recalled vials been prescribed? I had an unusual reaction in May after an injection. A blister came up on my stomach near the site, then I developed a red rash. My dr didn't think the methotrexate was the cause because I've been on it for years without a reaction. This is before the recall. The same thing happened again in July, I had to take predisone to clear it up. The scar from the blister is still there. My vials are on the recall list. Has anyone had a reaction like this?

my 10 year old daughter got diagnosed in late january of this yr with juvenile idiopathic arthritis/stills disease...she has been taking metho injections for awhile now and they werent helping so they added humira.. I see a difference sometimes..I just found this recall for the metho injections and now Im a very concerned..I dnt know if i should still give it to her...

I have had RA for 10 years and my Dr. (the head of the dept.at Mayo) said I should take the injectable methotrexate in juice in the eve. as it might cause nausea. I was in remission for 8 months(in 2004) but RA returned again and I was told to inject methotrexate in my thigh. I really did not this and the site where I injected was dark and when I went back to Mayo they said I could return to putting it in juice. I only take it once a week and just do 0.6 and have no more problems. I also take folic acid as metho can cause sores in the mouth. My understanding it also cheaper than pills. My 250 ml vial is $4.00 and it lasts for about 5 months.

MTX can cause lung problems. That is why most doctors will have their patients have chest xrays. It is not uncommon. That is a side effect of the drug--not becaue it's tainted.

I was on MTX for a long time but had to switch to Immuran because of the lung problems.

Why Megan's doctor didn't order a chest xray is unbelievable. Does that doctor read the side effects of the drugs s/he prescribes?

I am 27 and was diagnosed with RA at 25. I was put on methotrexate injections last December and shortly after became extremely sick. I was sick from the first week of December until June. In January and February I was so sick I thought I was going to die and I lost 30 pounds from throwing up daily. I could hardly breathe and after seeing 5 different specialist doctors a CT scan found a lung infection and the doctors still don't know what it is - I had been on antibiotics and it wasn't helping. I had to have 4 penicilin shots in my butt and was still extremely sick. I kept telling my RA doctor that methotrexate was causing it and he kept saying no it wasn't. In March I told him I want off the methotrexate now and he finally agreed. After going off of it it took me 3 months to feel better and after multiple CT scans my lung infection is finally gone as of October. 10 months of struggling to breathe!! It was a nightmare and I am terrified of ever injecting RA drugs into my body again and I will NEVER take METHOTREXATE for the REST OF MY LIFE!

I've taken 6 vials (.07 dosing) of affected lot since end of August 2010. I'm scared and angry at the same time. I HAVE felt flu like symptoms, prob. t.m.i. but bowel problems, exhaustion, swelling in legs, and chest pains--which may or may not be related. Only posting this for others with possible same symptoms. Have scheduled Dr. appt. and plan to investigate further today for what other possible actions I should take.

I too am upset about this methotrexate contamination. It is enough for my 16 year old son to have endured injections for the past 2 years, and now I am trying to figure out how much of this stuff we have
given him subcutaneously every week, might
have been contaminated with glass flakes?
Maybe none of it, maybe some of it??
Is the pharmacy company going to give us
the specific 4 lot numbers out of the 24 they recalled??

As far as I can tell, the risk was there over
a full one year period- that is a long time-52 weeks of injections!

I didn't keep my old vials, have one full box barely used that is on the recall list,
another full box totally consumed, over an 18 week period that is on the list, and then 3 or 4 other prescriptions going back the past year where I have no way of telling if they are recalled lots or not.....but I would assume so because of approximate expiration dates. I can't even figure out
based on when I had a prescription filled when it would expire.....one year, two years??

It is all a mystery, the pharmacy can't give me old lot dates, all I can get is the date
I filled the prescription.
people online say that the hot line isn't helpful...
it just adds insult to injury that when you think you are doing the best thing for your child and having to administer this powerful drug, that there is possible contamination.

I got the letter from the drug company and called our local pharmacy that fills my little ones methotrexate Was told they did not have any vials on recall. Thanks today to the notice from the Arthritis Foundation I checked again for the vial numbers and guess what, we have one that has been used. We had a hard time with this viall and Jessica cried. She bruised worse than ever and has had a blah feeling for quite a while. How many other vials have we used that were on recall?? that I do not have any more. When we called the manufacture today they offered to send me the recall package. I have decided NOT TO SEND IT BACK TO THEM. Because if we do, what proof do we have??? Especially since the pharmacy said they did not have any and here is the vial. Jessica has been on Metho since she was 18 months old, She is not 6 years old. She has enough other medical issues including a trach so now what else do we have to worry about. Has anyone found out what they are going to do about this??? What are the long term effects ??? Too many questions not enough answers any more

My niece takes metho for Ra, we call her pharmacy they told us that they didnt have any recalled vials, but they did we got a email, and checked it out. My niece is only 6 but I want to know if anything will happen in the end. She hasn't got her injection in a while but, she used the bottle and their is only half of it left, are they going to give us money for this or what are they going to do.

Kaiserpermanente supplied the metho that I have been injecting that is on the recall list. If I had not been alerted by the Arthritis Foundation, through my daughter, I still would not know about the recall. To date, no one from Kaiser has contacted me about the tainted drug batches. The pharmacy said they sent out a letter but I still have not received it. If I did not have a bad cold, I would have injected another vial.

My husband has taken a vial of the affected meth bottles and we are not sure when he has taken it being that he has been taking it for a year, we think that it was June because he is in constant pain we cannot tell whether after an injection there was any more pain then usual, does anyone how long the affects take to know that you have the glass particals? Is there a test or scan that can be done that will show if you infact have glass in your veins? I also have called Sandoz and was treated AWFUL really made me scared and helpless feeling
any information would be greatly appreciated

I have taken meth for 6 years, and my vials are on the list. I have 2 and a half left. The pharmacy I use hadn't heard of the recall. Thats the scarey part. I had also used Enbrel but that caused leukemia. So now I can only take meth and Rituxen, a cancer drug that is infused twice a year. That seems to be keeping my counts in check, even my white count which has dropped dramatically. Keeping my fingers crossed. I have a drug that gives a double whammy ! Good luck everyone.

I gave myself three of the injections of the recalled viels. I was contact from the pharmacy and pharmacutical company. They told me to contact my doctor which I did; however, my RA doctor was not aware of the recall, so they have not been much help. My concerns of what can happen to me. I realize that the glass can cause damage to blood vessels, swelling, and possible death. My question is how long can this be in one's body and cause problems. I have a call into my regular doctor, but have not heard back yet.

Does anyone know the effects of injecting the tainted meth I have used mine that is on the recall list and found it very hard to draw up and at the time could not figure out why but the inj site was bruised snd swollen and tender to touch but have been feeling dizzy and having headache since then Dont know if caused by that or something else

Thanks about the Methotrexate warening,i'm glad i take the pills,i didn't know there has been a a form of injections for methotrexate.Ihave been on the pills for 30 years and never had a side affect from it.I also take Humira shots every 10 days(40ml),Ihave server Rhumatoid arthritis foryears.I've been on so many ansaids,and steroids in years it is hurrendes.I've had courtisone shots (many)injections Yes it affects your bones a great deal,belive me.Just had shoulder surgery in Jan this year.Ready to have surgery for booth knees,now.Iknow what you are all going threw,some young so old..I fell bad for the young kids..thanks for the information on Remicaid i was going that direction but i'll have 2nd thoughts....

I'm on 8 tabs a week to combat my RA and did not even realize that there was an injectible form of methotrexate.

My question to pose out there is this, is there anyone out there who has had issues with the biologicals, Enbrel, Humira, Remicaid, or any of the new ones, Symponi, etc? I took 11 infusions of Remicaid and developed Stage 4 Breast Cancer, which has now shown up in a couple of spots in one bone and a couple of spots in my liver. My docs say they can't say for certain it gave me the CA but they are very much on board with the fact that it was highly likely the cause to accelerate the spread. I had absolutely no history of breast CA in the family so that's why we are leaning towards the meds as a cause.

I took Methotrexate for 5 years and got Kidney cancer, Renal Cell Carcinoma. Two years ago my kidney was removed, with 3-tumors inside it. Now this year cancer has come back in my bones.

i have been using Methotrexate for about 8 months 14 weeks ago i was switched to injections
on saturday 10/30/2010 i recieved a text message to my cell phone from my walgreens pharmacy informing of a recall of one of my medications
i spoke with a pharmacy tech i said recall & he said methrotrexate
the pharmacist informed it was a a level 1
high alert recall that is why as soon as the information was entered into their system that the aumated alert system started calling all listed users of this drug
walgreens is replacing the vials with vials from another manufacturer while i was typing this the pharmacl called & informed me that the replacement is ready

monday 11/25 was my last injection befor the known recall the injection site has been tender & itchy all week
11/18 the injection was very hard had to push extra hard on the plunger

well thats my 2 cents

I really appreciate the notice from Arthritis Today. Saturday I called WalMart about it as they dispensed some from the bad Lot #s to me and they denied carrying any of those recalled. Today, the phamacist told me that they did not have to inform me as the recall was not to the patient level. I looked again at the Sandoz media release and the recall is to the consumer/user level so that was wrong. I did get a replacement vial from her but am not happy with their lack of concern as I would have injected more of the bad Rx if you had not informed me of the recall.

Thank you for the notification. This is the only notice I have seen on this matter. I have checked my vials and found they are manufactured by Hospira of Illinois. I can take my injection tonight without concern since my supply is from a different manufacturer. Thank you for alerting me by email.

My wife took one vial of methotraxate a weeek ago. That vial is on the recall list. She has had much bruising around the area where she got the shot and can barely lift her leg. This was very puzzling to us and her Dr...but now not so much.

If you go on the website, there are four notifications. Letters to the patient, wholesalers, stores, etc. What disturbs me greatly is the letter to the individual about returning the vials and get them replaced....BUT no side effects mentioned. The letter to the vendors(Walgreen's, etc.)list many possible side effects.

I called the hotline number and they were very callous. They already have a legal department on hand to handle complaints. I am NOT returning anything until we get some answers.

I've had unremitting RA for over 40 years...That which doesn't kill me makes me stronger...boy am I strong...haha I've taken MTX injections for many, many years plus I take Enbrel injections...I think both meds help. This is the first time I've found my MTX on the recall list. I've taken at least 4 vials of recalled MTX so far over the past couple months...BIG Whoops...I will call Walgreen's Mail pharmacy first thing Monday. I found this recall notice by accident. I did not get notified by Walgreens's ~ yet.

Thank you for this fine article. I just recently stop the injection, because of nausea and other things, On October 18th, 2010.,was my last shot.I had been on this shot for quit sometime. It helped to apoint but started to get sicker while taking the injection. Also the pill did the same of nausea, but not as bad. My experience with both is the pill is my choice, got tired of the nurses injecting me the wrong way. But thank you for the info.

Connie, my word! These are not "vials of glass splinters", but vials which might contain a few small flakes of glass, clearly the result of some accident in the manufacturing process. The FDA can't monitor every batch of every drug that comes out of every drug company. This problem was probably found during the company's quality-control testing and the voluntary recall was issued as soon as it was found. They clearly mean to replace the drug that may be affected and I'm sure they will investigate any claims of injury. And if everyone who got the recalled vials calls a personal injury lawyer as Becky suggests, none of us will be able to afford the drug!

I have RA, and one of the meds I take to combat it is methotrexate. Mine is in pill form, not the injectable, but thank you for this article... it's pretty scary to think that it could harm people in that manner.

I have had rheumatoid arthritis for 22 years and I have been taking methotrexate injections for at least 17 years no problems. My vials were not on the recalled list. Thanks for the warning.

Fortunately, I haven't used my Methotrexate yet, due to a virus, but the vials that I have are not included in the recall. I received an email from drugs.com a couple of days before I received this email from arthritistoday, but actually found the article about the recall on google news at about the same time. I would contact a personal injury lawyer if I had taken the contaminated product.

How ironic....my husband's RA doctor @ the VA Hospital just presribed this injectable med replacing the pills just last week. We are expecting a delivery in the mail any day. I will check it as well as call the pharmacy for another drug. I will be afraid to use it at all. THANK YOU....GOD IS GOOD.

This looks like what I had for an injection in my heel a month or so ago.Every since my foot has been swollen.I am not sure if it is the same .What do you do about it if it is.Look forward to hearing back from you about this.

Thanks so much for the email--All 6 of my vials were on the recalled list (2 different lot numbers) and I was to use the first one tonight. I returned them to my pharmacy which had NOT heard of the recall yet--very disturbing! They replaced all 6 vials with fresh ones from good lots but I am so glad to be warned by this website. thanks for informing. never heard anything from my pharmacy. i had a batch of tainted methotrexate. they stated that their computers were down and were in the process of "getting out a letter". Meanwhile my son has had 3 doses already and 5 doses at home and i was going to give him one tomorrow. thank again.

Ingrid Fish: Are you really drinking the juice injected with your Methotrexate? I never heard of that option. Maybe you should verify this with your doctor. Any pharmacists or doctors out there who can verify this as an acceptable alternative to self injections?

I received a phone call yesterday from United Health Care that my injectable methotrexate is being recalled....a full day ahead of seeing it in the newspaper or receiving this notice. They already have replacement meds sent overnight and on the way to me. They will pick up my leftover recalled meds.

My last injection didn't really work as I was unable to draw the meds up into the syringe and didn't have another syringe. I suspect the vial was one contaminated with glass causing the problem. I have never had a problem in the 4 years I have been taking injectable. I am a retired nurse and ,yes, I do know how to draw meds into a syringe.

I found out in 1999 that I had rhumatoid arthritis and, knock on wood, have not had a flare up since about 2004. I have been on methodrextrate and hydroxychloro (plaqunel-may be spelled wrong) I have just recently had 2 infusions of Rituxan for RA. I have the 250 mg of methodrextrate but it also states 10 mL vial. it is from lot 204769 Exp 06/11 Do not see where it was mfg'd. Any concern?

I have 6 vials of the methotrexate from one of the batches that has been recalled. I have injected 6 from that batch so far. I did have swelling and bruising at the injection site which I have not had before. This is very disturbing.

Ingrid Fish, Yes take the tainted vials back to the pharmacy. You will get clean ons if they are in stock and if not in stock they will be there Monday. At least that is how Walgreen's is handling it.

I injected one of the vials and now have a bruise at the injection site. That is unusual so I am going to call me dr Monday.

Thank -you for the warning its nice to know you are on top of things. Although i don't take the injectible,I take the pills.I do take Humira every other wk. When I was on the injectios of Methotrexate my Dr. did the injections.I'm glad there is a site that cares.THANK_YOU!!!!!!!

Glad to have had notification about this problem. My dosage vial is not on the recall list.

I noticed several people asked if their medications were on the recall list including their oral pills. Please folks, there is a link directly to the list of the affected vials and the recall does not include oral pills.

By the way, if your doctor hasn't tried you with this combination and you aren't getting good enough relief ask them about MTX and Docycline. I've been on MTX and Docycline for the last year with good results, being as nowhere does our budget include money for those other drugs like Humira and Enbrel!

I have been on methotrexate for a number of years. At present I have 1 full bottle and a partial bottle but I do not believe that they have the Lot #'s that are recalled and the expiration date is Oct. 2011. I also recently picked up a new supply of the methotrexate by my Expiration date is April 2012. These both are a product of Hospira, Inc. out of Lake Forest, IL but do say Product of Australia.

Thank you for the warning notice. Any news on medications is a must.

I receive my meds from the VA and so far not a word from them. You would think they would be the first to know on something as serious as this. Unfortunately I have found the VA to be slack in notifying patients of any thing. Things have gotten worse since 2008 and the passage of OBAMANOCARE. Odds are against me hearing anything on this recall from the VA. Any Vets out there who have had notification please advise via my blog. afticker.blogspot.com. my concern is that ,is it only the injectable Methotrexate,how about the tablet is it safe,i've been taking it for almost 5 years now

I am taking Methotrexate, 0.8 ml once a week
injected into juice, and have to also inject Enbrel once a week in addition.

Kindly inform me whether I should return the
2 vials of Methotrexate on hannd to my pharmacist.

Please reply asap.

I do have 2 vials of the recalled lot #, and since they are delivered, I called the company that handles the prescription delivery; spoke with customer support and one of their Pharmacists. They arranged for the replacement of these 2 vials to be shipped to me and the pickup/return of the 2 recalled.

The vials I have are on the recall list, but my pharmacy know nothing about the recall. So now what?

Daughter is just starting injections this week-end, not the same mfg.

Lighten up Connie, doesnt mean he's glad anyone else is affected, he is fortunate his isn't.

I was diagnosed Oct.'08.I did not know there was an injectable metho. my body could not handle the pill version.I also did not know that RA atacks tissue as wellas joints I really need to do more homework on this disease.

You people feel "lucky" ??? because your vials did not contain flakes of glass in your Methotrexate injections?? What are your feelings toward those who received vials of glass splinters? It's not a matter of COST - injections of this drug can be life saving or life threatening (IF glass slivers are in your vials). Come on people, this is an inexcusable threat to life for a drug that is supposedly monitored by the FDA for safety!

We have at least 24 vials left for my son has been taking this for years now. Will we have to repurchase this when it's available again or will there be some sort of trade in? It's not one of his most expensive meds, but it's not exactly cheap either. Is there any word as to when it will be fixed? There are not any other meds work with his treatment like this does... How scary...
