Voltaren Gel Offers Rub-On Relief

Topical arthritis treatment may relieve pain with less risk of side effects. Is it right for you?

By Denise Lynne Mann

If you have arthritis in just one or two joints, you may not need to expose yourself to the risks of oral non-steroidal anti-inflammatory drugs (NSAIDs) – such as ulcers; stomach upset; or potential heart, kidney or liver problems – in order to achieve some relief. Rubbing an NSAID on your skin is now possible.

The FDA recently approved Voltaren Gel as the first prescription skin gel to treat osteoarthritis (OA) pain. The gel contains diclofenac sodium, an NSAID that is the main ingredient in Voltaren pills. (Read the latest FDA warnings on topical analgesics.)

A prescription topical NSAID is great news for select people with arthritis, says Roy D. Altman, MD, a professor of medicine in the division of rheumatology and immunology at the University of California, Los Angeles (UCLA).

Voltaren Gel may be a good option if:

 • You have arthritis in smaller joints. “The new Voltaren Gel gives patients the ability to apply something topically, which will not give significant blood levels, but will penetrate the skin and help reduce pain,” Dr. Altman says. “Voltaren Gel works as well as its oral predecessor when it comes to joints that are closer to the surface, such as the hands, knees, and perhaps the elbows and ankles.”

In the studies that led to the new gel’s approval, pain levels fell by 46 percent among people with hand OA after they applied Voltaren Gel for six weeks. In a 12-week study of people with knee OA, there was a 51-percent reduction in pain.

You are older than 65. “A lot of elderly patients can’t take oral NSAIDs because they have stomach or heart risk factors, and they can’t take narcotic analgesics because they could become so drowsy they could fall and break a bone,” Dr. Altman says.

 • You want to avoid pills. Some people with OA who want to avoid systemic side effects seek compounding pharmacies, so that they can have their favorite painkiller made into a topical formula. Pharmacists literally can take the medication out of the capsule and make it into a gel – voilà!

“This can be fairly expensive, however, and it can be inconsistent from one batch to another,” says Dr. Altman. “The new gel is less expensive and more consistent than a compounded topical formula. In addition, it comes with disposable dosing strips that show you exactly how much gel to use.” You squeeze the gel onto a card along the line for your dose, wipe the card directly onto your joint and rub it gently into the skin.

Voltaren Gel may not be a good option if:

You also take oral NSAIDs. It’s not that the gel’s active ingredient doesn’t get into your bloodstream – it does, Dr. Altman says. “Some does get absorbed, just substantially less than with a pill,” he says. Specifically, 94 percent less is absorbed from the gel than from its oral counterpart. However, the new gel should not be used in combination with oral NSAIDs or aspirin because of the potential for adverse effects. When used alone, the only real side effects of the topical products are skin reactions where they are applied.

You have several affected joints. “Voltaren Gel works fairly quickly – within a week – but the pill works quicker,” says Dr. Altman. And taking a pill would be a lot easier for someone who has multiple joints affected by arthritis, he says.

I have degenerative disk disease, and have had two apparently unsuccessful operations: a laminectomy in 2003 and a spinal fusion in 2005. I have tried every sort of physical therapy, injections, acupressure, acupuncture, massage, etc., to no avail. The only thing that abates the pain to a manageable level is a combination of MS contin and Endocet. Given that my pain is mostly emanating from my spine (and also dead center on my left buttock), neither of which area is a joint, per se, is it likely that this gel will be effective? Also, has anyone had experience with the kinesis tape (or similar name) often used by athletes?

I've had A.S. (Ankylosing Spondilitis) and Arthritis in other joints for 30+ years. At times the pain can be excruciating.

I've tried several rub-on creams and ointments over the years and the results have been ineffective. Personally, I've become pretty skeptical of claims of pain relief provided by many of the creams and ointments, claiming that a person can just rub it on their skin and have quick pain relief in the joints.

However, Voltaren Gel (diclofenac sodium topical gel) 1%, has actually produced noticeable pain relief in my hands, fingers and neck area. I'm very pleased with the results I've had from Voltaren Gel.

I hope to continue using Voltaren Gel in the future, combined with my other prescribed medications.

I'm one of probably thousands that find themselves between a rock and a hard place. Unfortunately I have been on blood thinner for quite a few years. I had knee surgery about forty tears ago now I arthritis in my knee well now I have been suffering with severe pain for almost two months now and have another month to go before I see my orthopedic specialist my primary care doctor says my knee is severely inflamed. Then she says can't give me anti-inflammatory pills or an injection because of my blood thinners and it could have a negative affect. So she gave me a prescription for pain. I take 12 different pills a day I don't need more pills. Why wouldn't she offer me this gel at least until I see the Orthopedic and what is he going to do I'm still on blood thinner, I imagine he will make me stop taking the blood thinner witch means I'll have to wait even longer. It's like a no win situation.
I'm also ticked-off that it takes the U.S.
forever to release of any new drug just so they can charge an arm and a leg to get. It's obvious they don't give a damn about us it's the almighty buck they care about.

More than 10 years ago on a trip to a resort in Mexico I began to feel pain in the shin area of one leg which I attributed to being out of shape and using fins while scuba diving and swimming, and walking alot on the beach. The pain was so severe I was unable to join a group of friends cycling into town. After bidding farewell to my buddies I inquired at the hotel's shops if they had a sports cream and was provided with a tube of Voltaren. Within 30 minutes of applying the gel the pain was gone. On a few occassions in the US I had attempted to purchase it at local pharmacies but was informed it wasn't available. On periodic trips to Mexico or Canada, rather than purchasing it via a Canadian Mail Order or Online pharmacy, I would purchase an extra tube or two, or one in a larger size. This product is great and it is about time it's available on the US market. Since it is available worldwide without a prescription, why must one have an Rx to purchase it here? (UNITED STATES).

Germany price for Novartis Gel is;
150gr for euro 8.90 ( about 12 dollars)

American price for 50gr( one third)
is $ 49,00 dollars on some sights.

my point once more;
Voltaren gel has been around for 20 years in Germany, relativly cheap, now it finaly is coming to the USA and is 3 times more money.
shame on you, Novartis , for ripping Americans off.
Shame on you America for beeing behind as usual.

I am so ANGRY at this country and it's ways I could scream. Voltaren Gel has been around for 20 years in Germany and proven to be save and succesfull.
Cost of the Novartis Gel in Germany:
150 gr. Euro 8.90 ( see internet AOL.DE)
In the USA it is $ 49.95 for 50 gr.
my point is this:
why again is the USA getting ripped off from Novartis? Why did it take 20 years to approve something Europe had that long?
Third World country, that is what we realy are.

Have had OA for more years than I like to remember. A few months ago my dr. prescribed a 4% voltaren in a gel. Expensive as I am in Canada. However, I have experienced some relief. It doesn't upset the stomach, and II haven't experienced any side effects, contrary to everything else I've tried.

I have been using Voltaren Gel for the last few months and I really appreciate Osteo-Arthritis pain being alleviated more by rubbing the gel on my upper arms, neck, lower back, and hips. I only use when necessary. This is so much easier on my body instead of popping more pills.

On the packaging and everywhere on the internet, it is stated that Voltaren Gel can cause serious stomach ulcers or bleeding, like an oral NSAID. Please explain how this can happen. It is a topical ointment...how is it that it can bother my stomach?

I have RA since 98 just came across voltaren. Had shoulder surgery/knee surgery 12/09. Started the voltaren 1 month before surgery on knee did not help BUT tried on joints on hand what a relief. My RA Dr said with all the med's I'm on, use it on my hands/ flare up spots ok. Keep a positive attitude some days are easier then others. I'm looking forward to getting back into my work out routine as you all know you have to keep moving no matter little or how slow you have to go. Good Day and keep smiling

I plan to try using Voltaren in addition to my usual routine of a couple applications of 0.25% capsaicin cream (Zostrix.

The capsaicin cream reduces pain by about 50%, and I'm hoping the Voltaren will knock out most of the rest of the pain.

To those who haven't used capsaicin cream or used it and didn't get good results: It is proven effective, but you have to use it consistently for several weeks to get full effects. And you have to be very careful with it. I not only had to heed all the warnings about it but also had to start with a very small amount and work my way up.

I may try pre-mixing concentrated capsaicin into the Voltaren if the latter seems to help.

I have arthritis all over my body, but when it started to be severe in my right hand (and I am right handed) I asked my doctor if there was anything I could do for it. I am already on several medications for arhtritis, but apparently my hand did't care. Voltaren Gel was suggested by my doctor and to my surprise, it worked. It didn't take any time at all. The first time I used it it helped releave the pain. No side effects either which for me is wonderful as I have problems with many drugs. Thanks heavens for this drug.

I have used one tube of Voltaren Gel RX over a period of time for OA in a few finger joints. The product worked well,I felt results in the first week of use 4 times daily. I continued use at 4 x daily and eventually backed it down from week to week as my OA swelling backed down. I still have joint damage and stiffness which will be there. But if I have another flair up I know I have my Voltaren Gel to get it under control.

I do have a question as to hair loss, with use of this Gel? I am a healthy 57 year old female with a thick head of hair, my beautician noticed a lot of hair breakage and hair loss/thinning on my forehead hair line. The only Rx I take is Lipitor RX and of course otc vitamines in addition to Voltaren Gel 1%. Has there been problems with hair loss or thinning with the use of this product?

I have moderate to (mostly) severe pain in various parts of my body, including severe degenerative disc disease in my lower lumbar spine, post surgical neurologic pain in my left knee after a total joint replacement, and moderate to severe pain on my left proximal and distal great toe joint and a cracked prosthetic implant installed about 5 years ago, just to name a few of the maladies I must accept to get through the day! I'm on a regimen of oral analgesics, but find the voltaren gel offers a moderate respite from some of the severe pain, particularly when rubbed in the area of my knee joints and great toe joint. It's not a panacea for all of my pain problems, but find it's efficacy is comparable to the use of Lidocane patches. It's the VERY best topical anagesic I've found, and man, I've tried 'em all!

Discuss with your physician to see if Voltaren gel would be appropriate for you, and if so, give it a try. There is currently a discount available on the "Voltaren" website that may make the product more affordable to you.

The best of luck, and may you have many pain free days!

I was just prescribed Volaren Gel and the Pill as well. I used the Voltaren Gel on the way home from the office and it was all I could do to make it to the bed for a long (2 1/2 hr.) nap. I do not think because of my heart and stomach problems that I will be able to use it. It really makes me sad because it immediately took away the pain.
I am somewhat scared of the patient info sheet and the warnings, especially after the
nap I had to take after using it.

voltaren Gel has *NOT* been discontinued. In fact, I just picked up my newest eight tubes today. I have to go to a special pharmacy, but I can get it. This product, combined with my opioid pain killers, gave me the life back that Lupus and Ankylosing Spondylitis stole from me. Now, I am able to stand in the courtroom for hours arguing on behalf of my clients. I appreciate it, and I know they do.
